Housing Case Manager

Position Purpose:  The Housing Case Manager is part of a service team, providing a safe haven for formerly homeless tenants to address barriers to self-sufficiency. This position works in a collaborative manner with the individual to accomplish common goals.  They will coordinate and monitor the assessment, treatment planning process and follow-up for each tenant of the agency's supportive housing and serve as an advocate and liaison for the tenant. 

Responsibilities:

  • Assist tenants in the development of individual plans to move toward self-sufficiency and to retain/secure temporary/permanent housing.
  • Complete new tenant assessment to determine eligibility and needs for social support programs. 
  • Evaluates, facilitates and monitors tenant's progress towards established goals and objectives.  Maintains and updates tenant information into agency's computer database system.

  • Provide resource/assistance for tenants in:  budgeting, job training, employment search, locating permanent housing, managing medical issues and addressing transportation and other needs.
  • Advocate for tenants with other service providers, including federal, state and city agencies and other non-profits; provide referrals to other appropriate service providers as needed. (e.g. Social Security, Medicaid, Medicare)

QUALIFICATIONS:

BA plus 3 years experience required,Master's Degree in human services preferred. (Social Work, Psychology) from an accredited college or university, with knowledge of issues of low income, homeless population, chemical dependency, and/or mental health programs. Ability to use computer software applications and general office equipment. Commitment to trauma informed care.

First Aid and CPR certification desirable. Maintain a valid driver's license and reliable transportation.

Ability to read, analyze and interpret governmental regulations and legal documents.  Ability to write reports and correspondence, Ability to speak effectively and respond to questions from tenants, management, and/or public groups. Bilingual skills desirable.
