Counselor, Safe Haven (Relief,) Greenfield

CSO is looking for Relief Counselors for our Safe Haven program in Greenfield. 

This counselor will be responsible for the care and treatment of clients, as well as specific tasks necessary to the operation of this program component. 

Responsibilities

  • Establish a 1:1 therapeutic relationship with each program client.
  • Provide support and safety for clients utilizing the program.
  • Assist clients with activities of daily living, including self-care, cooking, recreation, and medication responsibilities according to individual treatment plans.
  • Prepare daily evening meal for clients and staff when working the evening shift. Coordinate shift duties with other counselor working shift during meal preparation.
  • Function as an active member of the Treatment Team, on a day-to day basis, in the therapeutic setting including:
  • Attending staff meetings;
  • Rotating shifts, as needed;
  • Charting and reporting on daily records;
  • Maintaining knowledge of patient's treatment plan.
  • Monitor clients, including hourly bed checks and documentation, and provide closer observation as needed.
  • Share observations, interactions, and knowledge to develop 24-hour care plans.
  • Attend change of shift meetings and provide shift report on client activities and mental status.
  • Participate in ongoing in-service training for the purpose of maintaining, developing, and sharing counselor skills.
  • Attend regular supervision meetings with the Program Manager or his/her designee.
  • Make appropriate clinical referrals as part of case management duties.  Communicate regularly with family members, significant others, and involved providers in an effort to coordinate care.
  • Follow CCS/Respite procedures regarding medication administration and documentation.
  • Occasional food shopping and purchasing of supplies when needed for daily program operations.
  • Present cases to insurance companies and obtain prior approvals as assigned.
  • Follow all agency policies and procedures.
  • Provide client transportation as needed.

Qualifications

Clinical Bachelor's Degree with two years' experience working with mentally ill individuals or two years' of supervised internship or field placement experience at a human service agency preferred.  High school diploma with a minimum of five years' experience may be substituted.  Must have excellent oral and written communication skills, and computer proficiency including ability to work with email and word processing software.  Must be well organized, able to work independently, follow through with projects, and able to function as a productive member of a treatment team.  Must possess valid driver's license and reliable transportation.
