Family Partner, ICC/FIT, Pittsfield

CSO is looking for a Family Partner to join our Intensive Care Coordination team in Pittsfield. 

A Family Partner is a professional who draws upon their own lived experience as a parent or caregiver of a child with emotional, behavioral, or mental health challenges to support other families facing similar situations. They use their personal journey to build trust, foster hope, and help caregivers feel understood and less alone.

Family Partners play a unique role in bridging the gap between families and service providers. By sharing their perspective, they give families hope, practical tools, and confidence in navigating complex systems. They help ensure services are responsive, family-centered, and truly collaborative.

This is a peer position, for a candidate who has lived experience as a caregiver of a child who's received services for mental health diagnosis.

As a Family Partner:

You Will:

  • Work in conjunction with the Intensive Care Coordination (ICC) and Family-Based Intensive Treatment (FIT) teams to develop Individual Care Plans (ICP) and provide support for family members through process of assessment and Family Support Plan development; encourage involvement of natural supports for families
  • Oversee implementation of the Family Support Plan
  • Provide direct supports to family members
  • Share lived experience in a professional and supportive way to normalize challenges and reduce stigma
  • Work collaboratively with clinicians, care coordinators, and community providers to ensure the family’s voice is heard and respected in decision-making.
  • Deliver services on-site or in the parent/caregiver's home or community
  • Follow all provisions of service types, including grant recipient agencies
  • Provide education, coaching, training and assistance to parents in navigating child serving systems (DCF, DMH, juvenile justice)
  • Foster empowerment and building self-efficacy
  • Facilitate linkages to peer/parent support and self-help groups as necessary
  • Assist in identifying formal, natural support and community resources
  • Be flexible to work occasional evening and/or weekend shifts

Your Background:

  • Minimum high school diploma or GED and two years' experience working with children, adolescents or transition age youth
  • Experience as parent/caregiver of a youth with special needs, and preferably a youth with mental health needs; willingness to share this experience/journey with child serving systems
  • Valid driver's license and reliable transportation required
