CSO is looking for a Medical Case Manager in Northampton.
The Medical Case Manager will provide intensive
case management to adults and families needing such support to maintain their
independence and stability in the community, or who have been hard to engage in
the mental health system. These services will be provided both within the
outpatient clinic setting, group settings, and/or on an outreach basis, and may
be in provided in a supportive living site, apartment, family home, or
community setting.
As a Medical Case
Manager:
You Will:
- Serve as the agency liaison to support clients requesting
services. Ensure regular follow up and establish meaningful connections with
clients over time using a harm reduction, trauma-sensitive approach
- Collect health measurements, and screenings. Document, and
communicate results accordingly, supporting the physical health of the client
as part of holistic care provided at CSO
- Provide individualized support to clients in getting their
priorities addressed. Care Management priorities may include refill requests,
transportation requests, getting messages to care team members, providing
reminder calls, prompting messages, advocating for client with medical
providers and specialists, and providing updates back to the client when indicated
- Become proficient in teaching clients agency, including
problem-solving, communication skills, and conflict resolution. This includes
the identification and reinforcement in the use of natural supports
- Assist consumers with identifying appropriate mental health,
addiction and medical services and accessing services of choice. This
assistance is realized over time through continuous engagement, and may include
internal referrals within CSO, or external to trusted community partners, in
person, calls, or using telehealth
- Facilitate referrals to supports that help identify and access
entitlements when requested. Assist consumers with applications to community
agencies, such as, but not limited to, DMH, DDS, ACCS, SSDI, SNAP/DTA,
government cellphones, Regional Housing Authorities, MassHire Career Center
- Triage work as part of the continuum of care to prioritize close
collaboration and wrap-around approach for clients who may need intensive
short-term or ongoing long-term case management (clients accessing CSO ESP, or
urgent medication appointments).
- Support grant-funded activities in accordance with grant
guidelines
- Maintain complete, accurate and timely records. This includes
completing chart reviews to ensuring that Release of Information documents,
Medical Physical Exams, Discharge Summaries, and Labs are up-to-date. Check
MassPat when indicated, report any discrepancies to the nurse, and work to
update the items needed
- Provide counseling and intensive support to consumers during
psychiatric crises or periods of transition; receive cross-training and provide
back-up support to other team members as needed
- Provide transportation for consumers as appropriate, following
program guidelines
Your Background:
- Medical
Assistant Certificate or Associate's degree
- Computer proficiency
- Valid driver's license
and reliable transportation
