Transitional Care Coordination Supervisor, Gardner

Join CSO's Respite team as a Transitional Care Coordination Supervisor in Gardner.

This position will provide integrated case coordination to adults receiving Crisis Services having indicators of higher risk. Transition Case Coordinator Services emphasize follow up of individuals placed at inpatient level of care but may at times be expanded to other high risk profiles. Follow up is client centered, integrative and collaborative with the primary goal of creating successful linkages for the individual to appropriate and effective community supports. This position identifies the individual's transition needs and engages the individual in order to facilitate these linkages.

Qualifications

Bachelor's degree in a behavioral health discipline or related field, or equivalent combination of high school diploma or Associate's degree and applicable experience. Peer experience may be considered as applicable. Demonstrated experience working with adults with mental health challenges including experience navigating the behavioral health system.  Good listening skills; ability to foster self-sufficiency of individuals using problem-solving and practical skills; familiarity with support groups or education groups. Excellent oral and written communication skills, and computer competency.  Must be able to work independently and follow through with projects as well as be an effective member of a team.  Must be able to work flexible schedule which may include evening and weekend work, as required by individual needs. Must possess valid driver's license and reliable transportation.
