CBHC Case Manager, Leominster

CSO is looking for a Case Manager for our Community Behavioral Health Center (CBHC) in Leominster. 

This position will provide case management to people of all ages needing assistance with navigating community resources that are available to them. The CBHC Case Manager provides services on an outreach basis which may include a supported living site, apartment, or family home.

Starting salary: $38,000-$44,000/year

As a Case Manager:

You Will:

  • Provide skills training and assistance in areas of daily living activities including basic living and interactive skills. Assist in developing communication and conflict resolution skills
  • Provide counseling and intensive support to consumers during psychiatric crises or periods of transition
  • Assist consumers with identifying appropriate mental health, addiction and medical services and accessing services of choice
  • Assist with developing and maintaining a "natural support network" of family and friends.
  • Assist in locating acceptable housing, negotiating leases, and obtaining furniture and other necessary fixtures
  • Be responsive to client requests, flexible and timely in providing services, creative in finding solutions, and dedicated to a person-first approach to care
  • Assist in accessing and completing documentation to improve/support independent living, such as obtaining a birth certificate, driver's license, financial aid, etc.
  • May provide recovery support services including exploring treatment recovery options with consumers, developing a set of goals and objectives in conjunction with the consumer based on identified needs, providing education and resources, and assisting consumers in accessing treatment and community supports
  • Provide transportation for consumers as appropriate, following program guidelines
  • Maintain appropriate contacts with family members, others in the "natural helping network," outpatient therapists, and others as necessary to support consumers
  • Complete NOMS, Quarterly Health Assessments and other required documentation in a timely manner
  • Attend in-service trainings on evidence-based best practices and other topics as appropriate; participate in CSO training and education around trauma informed care and participate in agency-sponsored quality improvement initiatives
  • Ensure record keeping and service delivery standards meet CBHC quality metric requirements, as determined by the CBHC contract

Your Background:

  • Bachelor's degree in a behavioral health field
  • Excellent organizational and interpersonal skills
  • Valid driver's license and reliable transportation
