CSO is looking for a Case Manager Supervisor for out CBHC PACT Program (Community Behavioral Health Center/Program of Assertive Community Treatment)

As part of a multi-disciplinary treatment team, provide support to Case Management staff and intensive case management to adults 18 years of age and older needing such support to maintain their independence in the community, or who have beforen hard to engage in the behavioral health system.? The CBHC PACT Case Manager Supervisor provides oversight to the team of PACT Case Managers as well as provides support services on an outreach basis and with the purpose of enhancing the emotional well-being, stability and social functioning of program consumers.

You will:

• Provide programmatic and administrative day-to-day supervision for PACT Case Managers in identifying clients needing case management (CM), focusing support on client treatment plan and goals, and working within the PACT model to support service delivery to clients in the community. This may include client transportation.

• Provide skills training and assistance in areas of daily living activities including basic living and interactive skills, providing "in vivo" skills training, side by side assistance, vocational and pre-vocational support to clients to include identifying interests and strengths and goals, and development of individualized placements. Assist in developing communication and conflict resolution skills.

• Provide motivational counseling, prompting, and intensive support to consumers during psychiatric crises or periods of transition; monitor and assist clients with personal hygiene and grooming tasks, grocery shopping and meal preparation, and household activities.

• Provide education and support with identifying community resources including appropriate mental health, addiction and medical services, and accessing services of choice.

• Provide assistance with developing and maintaining a "natural support network" of family and friends.

• Provide crisis and intervention treatment services as members of mini-treatment team.

• Assist, train and support clients in locating and maintaining decent and affordable housing, negotiating leases, and obtaining furniture and other necessary fixtures; facilitate cooperative relationships with landlords and housing authority as indicated.

• Act as member of mini-treatment team in accordance with treatment goals and planned activities/interventions and effectively communicate with other team members regarding client care. 

• May provide recovery support services including exploring treatment recovery options with consumers, developing a set of goals and objectives in conjunction with the consumer based on identified needs, providing education and resources, and assisting consumers in accessing treatment and community supports.

• Ensure record keeping and service delivery standards meet program quality metric requirements, as determined by the CBHC/PACT contract.

• Provide transportation for consumers as appropriate, following program guidelines.

• Maintain appropriate contacts with family members, others in the "natural helping network," outpatient therapists, and others as necessary to support consumers.

Your Background:

Bachelor's degree in a behavioral health discipline or related field.

Experience working with adults with mental health challenges including experience navigating the behavioral health system

Excellent oral and written communication skills; computer proficiency. Must be able to work flexible schedule which may include evening and weekend work, as required by client needs.

Valid driver's license and reliable transportation required.
