CSO is looking for a Nurse to join our
CBHC (Community Behavioral Health Clinic) PACT (Program of Assertive Community
Treatment) program in Florence.
Program for Assertive Community
Treatment (PACT) is an evidence-based intensive outpatient treatment program
designed to assist youth overcome the barriers to their recovery from severe
and persistent behavioral health disorders.
This position will provide nursing assessment,
evaluation, monitoring, treatment planning and coordination of care and will
provide treatment, rehabilitation and support services with the purpose of
enhancing the emotional well-being, stability and social functioning of program
consumers.
As a
PACT Nurse:
You
Will:
- Conduct nursing visits to assigned
consumers in their homes or other residential settings. Visit shall
include brief assessment and evaluation, provision of planned care, and
patient education and support to assist them with self-management of their
care; provide "in vivo" skills training, side by side assistance,
supervision, prompting, motivational counseling and environmental
adaptations to assist clients with activities of daily living
- Provide individual and group therapy and psychoeducation
in office and community settings.
- Monitor Primary Care/health needs of clients and provide
referral, advocacy as needed to promote optimal health
- During outreach visit, communicate with family members or
other caregivers regarding the patient's care as appropriate and permitted by
the patient
- Following (or during) outreach visit, coordinate with
team members as appropriate regarding further care and support for the consumer
- Provide stage specific individual and group substance
abuse treatment to clients with coexisting substance use disorders
- Provide crisis and intervention treatment services as
members of mini-treatment team.
- Act as member of mini-treatment team in accordance with
treatment goals and planned activities/interventions and effectively
communicate with other team members regarding client care
- Maintain current treatment plans for all assigned clients
- Under direction of PACT Psychiatrist, supervise
medication-related activities of team, working with team and clients to monitor
medication effectiveness and side effects; develop policies and procedures
related to medication management and completed required documentation
- Organize, manage and administer medications for clients
as indicated. Maintain pharmaceutical supplies and documentation in accordance
with DPH regulations
- Act as member of mini-treatment team in accordance with
treatment goals and planned activities/interventions; participate in Care Teams
as appropriate for individual consumers needing ongoing outreach
- Collaborate with team members to ensure optimal planning
and support for consumers, participate in development of transition plans, and
provide transition support activities as needed
Your
Background:
- Licensed as a Registered Nurse in
Massachusetts
- Experience in
clinical or rehabilitation setting with individuals with major mental illness
- Computer
proficiency
- Valid driver's
license and reliable transportation
