Relief Integrated Care Nurse, Pittsfield

CSO is looking for a Relief Integrated Care Nurse.?

This Nurse will provide medical care and treatment for clients, ensuring coordination of care and appropriate discharge planning.

As a Relief Integrated Care Nurse:

You Will:

  • Provide medical care and monitoring for program clients, including medication order fulfillment and administration

  • Contribute to the Comprehensive Assessment and care planning of engaged program clients, including evaluating for substance use and withdrawal

  • Assist in the evaluation of clients in need of emergency treatment services for risk management, safety planning, and possible hospitalization, providing crisis counseling and crisis prevention planning as needed

  • Contribute to the development, adaptation and execution of transition plans, including medication reconciliation process as appropriate

  • Provide in-service training for staff on common medical issues and appropriate treatment, intervention and support strategies as well as provider psycho-education

  • Conduct medication reviews and monitoring as needed

  • Complete necessary reports and paperwork in a timely manner

  • Attend in-service trainings on evidence-based best practices and other topics as appropriate

Your Background:

  • Must be licensed in Massachusetts as a Registered Nurse?

  • Valid driver's license and reliable transportation
