PACT Nurse, Northampton

CSO is looking for a Registered Nurse to join our PACT team in Northampton.

Program for Assertive Community Treatment (PACT) is an evidence-based intensive outpatient treatment program designed to assist adults overcome the barriers to their recovery from severe and persistent behavioral health disorders. The program operates with a strength-based and person-centered approach and is targeted to individuals with severe behavioral health disorders who exhibit continuous high service needs, who have experienced multiple inpatient psychiatric hospitalizations, and who have been unable to benefit from more traditional outpatient behavioral healthcare.

The PACT Nurse will provide clinical assessment, evaluation, monitoring and education to adults (18 and older) on an outreach basis in homes, other residential, or community settings. The nurse in this position will participate in Care teams and Care Plan development, provide consultation to other PACT staff as needed, and assist in communication with medical providers, members and families as needed. They will collaborate to ensure optimal planning and support for consumers, participate in development of transition plans, and provide transitional support activities as needed while providing outreach to other clients as assigned.

Responsibilities

  • Conduct nursing visits to assigned consumers in their homes or other residential settings. Visit shall include brief assessment and evaluation, provision of planned care, medication reconciliation, and patient education and support to assist them with self-management of their care.
  • During outreach visit, communicate with family members or other caregivers regarding the patient's care as appropriate and permitted by the patient.
  • Following (or during) outreach visit, coordinate with PACT team members as appropriate regarding further care and support for the consumer.
  • Participate in Care Teams as appropriate for individual consumers needing ongoing outreach.
  • Provide in-service training for PACT staff on common medical issues and appropriate treatment, intervention and support strategies.

Qualifications

Current Massachusetts license as Registered Nurse. Experience treating individuals with serious mental illness and substance use disorders preferred. Familiarity with community resources strongly preferred. Must possess good communication and interpersonal skills and the ability to establish trusting relationships through compassion, empathy and insight. Capacity to be an effective member of a Team. Computer proficiency required. Valid driver's license and reliable transportation required.
