Care Coordinator, CBHI, Greenfield

CSO is looking for a Care Coordinator to join our ICC team in Greenfield. 

Become a certified, wraparound facilitator of the Intensive Care Coordination Program.  The Care Coordinator facilitates a care planning process based on the youth and family's needs and collaborates with state agencies, school systems, primary care physicians, and family supports to form the family's team.  The Wraparound Process is family driven and team informed and the goal is to create an integrated care plan.

Responsibilities  

  • Facilitate care planning team (CPT) meetings.
  • Develop and implement a youth and family-centered intensive care plan (ICP) in collaboration with the family and collaterals. 
  • Develop early and ongoing team collaboration with a Family Partner who provides family support and training to parents and caregivers. 
  • Face-to-face contact with the youth and family, as determined by the youth, family and members of the CPT.
  • Provide referrals and linkages to appropriate services along the continuum of care.
  • Coordinate and plan for multiple services and supports. 
  • Help caregivers learn effective advocacy skills. 
  • Identify and help develop natural supports.
  • Maintain regular contact  with the family, youth (where appropriate) and other relevant persons in the youth's life.

Qualifications

(One of the following)

Master's degree in a mental health field (including, but not restricted to, counseling, family therapy, social work, psychology, etc.) from an accredited college or university 

Bachelor's degree in a human services field from an accredited academic institution and one year of relevant experience working with families or youth.  If the bachelor's degree is not in a human services field, additional life or work experience may be considered in place of the human services degree

Associate's degree or high school diploma and a minimum of five (5) years of relevant experience working with the children age 3-21 with emotional/physical challenges, experience in navigating any of the child/family-serving systems and experience advocating for family members who are involved with behavioral health systems

Demonstrated experience working with children, adolescents and families, including experience navigating children's systems and supports within the community.  Knowledge of and training in child development and children's mental health experience. Good listening skills, nonjudgmental ability to foster self-sufficiency; desire to work in natural family and youth settings.  Ability to establish trusting relationships through compassion, empathy and insight. Demonstrated sensitivity to the needs of families from diverse cultural or linguistic backgrounds.  Computer proficiency required including ability to work with email, electronic health record and word processing software.
