Enrollment and Data Manager, CP, Greenfield

CSO is looking for an Enrollment & Data Manager for our Community Partner Program in Greenfield. 

The Community Partner Program is an innovative service provider with a multidisciplinary team consisting of nurses, Licensed Clinical Behavioral Health Specialists, Bachelor's level Care Coordinators, and Community Health Workers.  Together we provide individualized support to adult MassHealth members who have complex medical and behavioral health needs. We coordinate communication between behavioral health, substance use, and medical care providers, and we connect people to services in the community to help them meet their self-identified health and wellness goals.

Responsibilities

Provide overall administrative support including overseeing the Enrollment and Outreach processes for adults referred to the CP Program, maintaining relationships with external stakeholders, supervising the Program Assistant and ensuring compliance with all aspects of the Mass Health contract including data management/metrics and support managing towards program outcomes.  The Community Partner program works with adults with co-occurring serious Mental Health/Substance Use Disorder and Medical Conditions to coordinate care to improve health outcomes.  The CP team is a multidisciplinary team with behavioral health and nursing supports. 

Qualifications 

Bachelors' Degree and 3-5 yrs. office experience, data management experience as well as customer service or client/patient direct care experience. Supervisory experience preferred.  Experience with behavioral health setting and/or medical records also preferred. Must possess excellent organizational and interpersonal skills, and the ability to handle situations in a professional manner at all times with clients, staff and external sources. Will possess strong interpersonal skills, including patience and a nonjudgmental approach. Will have ability to adapt to changing needs and responsibilities. Computer proficiency required including ability to work with email, electronic health record, data bases and word processing software. Ability to manage multiple projects. The ideal candidate will have demonstrated practice that aligns with CSO's values of Trauma informed Care Principles and Cultural Responsiveness.
