Office-Based, Behavioral Health Nurse, Athol

CSO is looking for a Nurse to provide medical care and treatment for clients, ensuring coordination of care and appropriate discharge planning.

This position in a Monday-Friday 1st shift position- no nights, no weekends, no major holidays, and no on-call requirement! 

Responsibilities

  • Provide medical care and monitoring for program clients, including medication order fulfillment and administration.
  • Contribute to the Comprehensive Assessment and care planning of engaged program clients, including evaluating for substance use and withdrawal.
  • Assist in the evaluation of clients in need of emergency treatment services for risk management, safety planning, and possible hospitalization, providing crisis counseling and crisis prevention planning as needed.
  • Contribute to the development, adaptation and execution of transition plans, including medication reconciliation process as appropriate.
  • Provide in-service training for CBHC staff on common medical issues and appropriate treatment, intervention and support strategies as well as provider psycho-education.
  • Be available to work flexible schedule which may include evening work and availability by phone to supervise and assist with plan implementation, as required by the needs of the program.
  • Conduct medication reviews and monitoring as needed.
  • Complete necessary reports and paperwork in a timely manner.
  • Attend in-service trainings on evidence-based best practices and other topics as appropriate.
  • Ensure record keeping and service delivery standards meet CBHC quality metric requirements, as determined by the CBHC contract.
  • Comply with DMH, DPH and other licensing authorities, and third-party payor regulations regarding record-keeping.
  • Assist with discharge planning and collaborate with staff from other programs and agencies in order to coordinate care.

Qualifications

Must be licensed in Massachusetts as a Registered Nurse, and have experience treating adults, adolescents and children.  Must be familiar with community resources and possess good communication and interpersonal skills.  Computer proficiency required including ability to effectively work with email, electronic health record and word processing software. Must possess valid driver's license and reliable transportation.
