Wellness Coach CCBHC (Gr)

CSO is looking for a Wellness Coach to join our Certified Behavioral Health Clinic team (CCBHC). The wellness coach will implement and maintain comprehensive wellness programming services to clients within the CCBHC program.  They will establish a link to primary health care and health promotion activities. They will support the establishment of a health culture within all CCBHC programming, groups, and program sites by assisting clients with reducing risk behaviors. They will inform clients of the risks associated with risk factors including, but not limited to smoking, poor illness self-management, inadequate nutrition, infrequent exercise and inadequate financial health behaviors.

Responsibilities include:

  • Develop a wellness curriculum with assistance and consultation by the Wellness Program Manager, which will be implemented by the Wellness Coach in both individual and group formats with CCBHC clients
  • Conduct group workshops and trainings to clients to provide education of wellness-related topics based on the wellness curriculum components
  • Collaborate with CCBHC team members in the development of treatment plans for clients
  • Participate in individual and group supervision

Qualifications:

  Bachelor's Degree or High School Diploma/GED and preferred 2 years of experience working with people with serious mental illness, addiction disorder and/or physical health problems

  •   Certified as an inSHAPE trainer preferred
  •   Good oral and written communication skills
  •   Computer proficiency
