IHT and Intensive Hospital Diversion (IHD) Clinician, Gardner

CSO is looking for an IHT and Intensive Hospital Diversion (IHD) Clinician for our Clinic in Gardner.

To provide a structured, strength-based therapeutic relationship for children and their families in natural family settings such as home or school in collaboration with a paraprofessional who provides family support and training as part of the clinician's treatment plan (such plan serving as either the hub of a family's services or as part of an Intensive Care Coordination care plan).

  (Intensive Hospital Diversion) To provide intensive, short-term in-home crisis stabilization and treatment to youth and their families to support diversion from psychiatric hospitalization and other out-of-home placements.  To provide intensive treatment and support needed to maintain the youth at home safely and to (re)connect them to ongoing outpatient and/or community-based treatment.

Responsibilities
  • Available to respond to a family at the point of crisis within 24 hours of referral to Intensive Hospital Diversion Program.
  • Coordinate provider meetings timely to meet each service's specification guidelines.
  • Respond to enrolled families 24/7, including in-person, when clinically indicated for Intensive Hospital Diversion Program.
  • Intensity of in-person sessions will match each service's specification guidelines.
  • Begin initial assessment for all referrals within 24 hours of referral, including a mental status and risk assessment of the referred child and family situation.
  • Develop treatment plans and goals in collaboration with families, children and other supports and providers.
  • Assist caregivers in stabilizing the behavior of children and adolescents, including the development of Risk Management Plans and facilitating the involvement of Crisis Services when required.
  • Be available to work flexible schedule which will include some evening and weekend work, as required by client needs.
  • Work collaboratively with the Family Support paraprofessional.  Be a collaborative provider to the family and the family's wrap-around team supports and providers.
  • Work periodic on-call shifts providing mobile crisis intervention response under the CSO Crisis Services Program (if required). This will require training in Crisis Services and Mobile Crisis Intervention procedures.
  • Maintain complete, accurate and timely records.
  • Maintain regular and required communication with MassHealth MCE's and commercial insurance providers.
  • Follow standard procedures as defined in the specifications for In-Home Therapy under the Children's Behavioral Health Initiative.
  • Follow standard procedures as defined in the specifications for Intensive Hospital Diversion.
  • Meet and maintain expected productivity level.

Qualifications

Master's Degree in Psychology, Social Work, Counseling, or closely related field from an accredited program. Demonstrated experience working with children, adolescents and families, including experience navigating children's systems and supports within the community.  Knowledge of and training in child development and children's mental health experience. Good listening and parenting skills, nonjudgmental ability to foster self-sufficiency in families using problem-solving and practical skills; familiarity with support groups or parent education group.  Ability to establish trusting relationships through compassion, empathy and insight.  Be an effective member of a Team.  Demonstrated sensitivity to the needs of families from diverse cultural or linguistic backgrounds. Computer proficiency required including ability to work with email, electronic health record and word processing software.
