Transitional Case Coordination Supervisor, Gardner

Join CSO's Respite team as a Transitional Case Coordination Supervisor in Gardner.

This position will provide integrated case coordination to adults receiving Crisis Services having indicators of higher risk. Transition Case Coordinator Services emphasize follow up of individuals placed at inpatient level of care but may at times be expanded to other high risk profiles. Follow up is client centered, integrative and collaborative with the primary goal of creating successful linkages for the individual to appropriate and effective community supports. This position identifies the individual's transition needs and engages the individual in order to facilitate these linkages.

As a Transitional Case Coordination Supervisor:

You Will:

  • Provide programmatic and administrative day-to-day supervision for the Transitional Case Coordination service
  • Provide initial engagement of higher risk individuals either prior to inpatient placement or as soon as possible after placement
  • Develop transition plans based on person's unique profile including the individual's support system, provider recommendations, and needs identified by the individual
  • Provide outreach to the individual and their providers by telephone and in person
  • Complete necessary documentation, releases, and other paperwork
  • Provide assistance with clarifying transition needs and identifying potential gaps
  • Support a comprehensive approach in transition planning that identifies integrative needs including physical, medical, and emotional wellness
  • Consult with the clinical supervisor on safety issues, barriers to transition, and questions about effective interventions
  • Provide assistance with identifying and using natural supports including family and friends
  • Assist in locating acceptable housing, transportation to appointments, and utilization of supports for transition
  • Collaborate with individual's providers as needed
  • Remain engaged with individual through transition and then complete steps to close services at that point. Consult with clinical supervisor prior to ending support services

Your Background:

  • Bachelor's degree in a behavioral health discipline or related field, or equivalent combination of high school diploma or Associate's degree and applicable experience
  • Peer experience may be considered as applicable 
  • Valid driver's license and reliable transportation
