Housing Support Case Worker, Northampton

CSO is looking for Housing Case Managers to join our Low Threshold Housing and Services team. We have multiple openings within Hampshire, Franklin, and Hampden counties.

This position will work in contact with referral sources, provide screenings, offer information and education, and help clients to manage the logistics of their substance abuse treatment. Also, this role will provide direct navigation and focus on clients' engagement, retention, and readiness to change. Services will be provided on an outreach basis.

As a Housing Case Manager:

You Will:

  • Provide assistance in locating acceptable housing, and obtaining furniture and other necessary fixtures
  • Manage an average caseload of 10-12 tenants
  • Provide transportation to tenants as appropriate
  • Maintain appropriate contact with clients, and other service providers such as: outpatient therapists, PCPs, and others as necessary to support tenants
  • Support in creating individualized service plans to guide tenants
  • Facilitate "mediation" as appropriate and provide counseling and support during crises or during times of transition
  • Assist tenants with finding appropriate mental health and medical services
  • Provide assistance with developing and maintaining a "natural support network" of family and friends

Your Background:

  • Bachelor's degree in behavioral health discipline or related field, OR Associate's degree and 2 years of applicable experience
  • Demonstrated experience in Case Management, preferably working with vulnerable populations
  • Valid driver's license and reliable transportation (required)
  • Efficiency working with computer programs and other office-based tools
