Housing Navigator/Recovery Specialist, Medical Respite, Northampton

CSO is looking for a Housing Navigators/Recovery Specialists for our Medical Respite program in Northampton.

The Housing Navigator/Recovery Specialist assists in accessing support services and making referrals to and advocacy with CBOs on behalf of the client. They provide substance abuse education and direct connections to treatment, and provide extended aftercare.

Responsibilities

  • Work with individuals identified as a high priority to coordinate services and get them housing ready such as documents, ID and income verification and assist with housing search and placement.
  • Access the data system daily to obtain and manage caseload and keep up-to-date client records.
  • Assist individuals in gathering all documents necessary to obtain permanent supportive or other housing.
  • Assist individuals in accessing mental health, addictions and primary care services.
  • Provide substance abuse education.
  • Support Individual Service Plan development and make case presentation to MDT.
  • Participate in assessments as needed with clients to ensure proper supportive services are provided.
  • Participate in and prepare for weekly meeting including staff meetings, case conferencing and other related meetings and provide regular feedback to Program manager.
Qualifications

Associate's or Bachelor's degree in social work or other related field or relevant experience. Two years' work experience in housing search and/or case management. Demonstrated ability to work independently or as part of a team. Strong written and verbal communication skills. Problem solving and conflict resolution skills. Computer skills with proficiency in Microsoft office programs. Ability to work with diverse population and exercise good judgement.
