RN Integrated Care Supervisor, Gardner

CSO is looking for a Nurse to be an Integrated Care Supervisor at our clinic in Gardner. 

Join our team as an Integrated Care Supervisor. Provide direct care as well as supervision and leadership to program staff related to integrated care within and across programs. This role will assist with the development of a wellness curriculum and associated wellness groups You'll provide clinical screening, evaluation, monitoring and education in a clinic and community setting.

As an RN Integrated Care Supervisor:

You Will:

  • Provide programmatic and administrative supervision of program staff as assigned
  • Collaborate with team members in the development of treatment plans for clients
  • Provide clinical screening, evaluation, monitoring and education to clients
  • Conduct group workshops and trainings to clients to provide education on medical topics
  • Assist prescribers as needed when administering suboxone
  • Administer medication as needed to clients in CSO programs and provide cross-program coverage as appropriate and in consultation with supervisor
  • Provide in-service training for staff on common medical issues and appropriate treatment, intervention and support strategies
  • Provide case consultation pertaining to medical issues for staff as requested
  • Communicate with CSO Prescribers and staff (e.g., hospital staff, PCPs, nursing home staff) as needed to support and advance appropriate care for clients
  • Conduct medication reviews and monitoring as needed
  • Assist with the development of a wellness curriculum and the creation of wellness-related groups.

Your Background:

  • Licensed in Massachusetts as a Registered Nurse or LPN
  • Experience treating individuals with serious mental illness and substance use disorders (preferred) 
  • Primary care experience (preferred) 
  • Valid driver's license and reliable transportation
