Housing Case Manager, FOH, Springfield

CSO is looking for a Housing Case Manager to join our team at Friends of the Homeless in Springfield. 

The Housing Case Manager coordinates and monitors the assessment, treatment planning process and follow-up for each tenant of the agency's supportive housing arrangements. They serve as an advocate and liaison for tenant benefits.  The Housing Case Manager is part of the agency service team, providing a safe haven for tenants to address barriers to self-sufficiency. They works in a collaborative manner within/outside department to accomplish common goals and ensure consistency of interpretation of policies/procedures and guidelines for tenants.

Responsibilities

  • Assist tenants in the development of individual plans to move toward self-sufficiency and to retain/secure temporary/permanent housing.
  • Complete new tenant assessments to determine eligibility and needs for mental health, medical, substance abuse, and other social support programs. 
  • Evaluate, facilitate and monitor the tenant's progress towards established goals and objectives. 
  • Maintain and update tenant information into agency's computer database system.
  • Provide resources and assistance for tenants in areas such as budgeting, job training, employment searches, locating permanent housing, managing medical issues and addressing transportation and other needs.
  • Advocate for tenants with other service providers, including federal, state and city agencies and other non-profits and provide      referrals to other appropriate service providers as needed. (e.g. Social Security, Medicaid, Medicare)

Qualifications

Associate's degree or equivalent from two-year college, or three to five years related experience. May also have an equivalent combination of education and experience working in the Human Services Field, such as working with low income and homeless populations, chemical dependency and/or mental health programs sufficient to successfully perform the essential functions of the job.  Ability to use computer software applications and general office equipment. Must have valid driver's license and reliable transportation.
