Case Manager, FOHITS Program, Springfield

CSO is looking for a Case Manager to join our team. The case manager for the friends of the Homeless Integrated Services Treatment program assists in accessing support services; makes referrals to and advocacy with CBOs on behalf of the client; provides substance abuse education and direct connections to Tx; provides extended aftercare; supports ISP developments and makes case presentations to MDT.

The Case Manager will be responsible for working with homeless individuals identified as high priority to coordinate services needed to get them housing ready. He/she will assist homeless individuals in accessing mental health, addictions and primary care services, provide substance abuse education, and will support ISP development and make presentation to MDT. The Case Manager will participate in assessments as needed and regularly attend staff meetings and case conferences. 

Qualifications: Associate's or Bachelor's degree in social work or other related field or relevant experience. Two years' work experience in housing search and/or case management. Demonstrated ability to work independently or as part of a team. Strong written and verbal communication skills. Problem solving and conflict resolution skills. Computer skills with proficiency in Microsoft office programs. Ability to work with diverse population and exercise good judgement. Must have valid driver's license and reliable transportation.

Preferred: Knowledge of local social service and other community resources. Familiarity with Hampden County Continuum of Care. Bilingual Skills (English-Spanish).
